Unzip
Download the ZIP and unzip it. You get a folder named MeasureTool.

Open the preferences folder
In Cinema 4D, go to Edit → Preferences and click "Open Preferences Folder..." at the bottom. Then close Cinema 4D.

Copy the folder
Open the plugins folder there (create it if it's missing) and copy MeasureTool into it.
...\plugins\MeasureTool\measuretool.pyp

Start Cinema 4D
Find it under Extensions → Measure Tool, or press Shift+C and type "Measure Tool".
Not showing up?
Check that the folder isn't doubled after unzipping (MeasureTool\MeasureTool\...). Error messages appear under Extensions → Console. To update, replace the folder. To uninstall, delete it.
